In two weeks, on Tuesday June 27, 2023, from 3-6 pm, FHN will be hosting sports physicals at the school. This will take place in the Field House (the elementary gymnasium). Cost is $25, cash or check made out to FHN only.
Any student athlete is welcome, and no appointment is necessary. However, students must complete and have a parent/guardian sign the forms below (forms will also be available at the school).
Remember, the sports physical meets IHSA eligibility requirements for all sports, but it does NOT meet the requirement for a 6th or 9th grade physical, or for new students from out-of-state. (However, those full physicals are accepted for sports requirements.) For more information about health requirements, see the Health Information page of our website.
If your student will be getting the sports physical, he/she should enter at the elementary end of the school. If you cannot make it to the Orangeville site, FHN is giving sports physicals at many area schools. See their website for more information https://fhn.org/sports-physicals-area.asp
