Athletic Forms

There are several required items that must be completed before a student athlete in grades 6-12 can participate in their first practice:

  1. Student must be fully registered for the school year they wish to compete, and the Athletics Underclassmen Waiver and Sports Medicine Consent portion must be completed. Orangeville does this electronically via parent portal each year.

  2. A current IHSA sports physical form (6th & 9th graders may use the regular required school physical in place of a sports physical) must be turned in to the office or athletic director.

  3. An Athletic Emergency Plan completed and signed by parent & student for the current school year must be on file with the athletic director.

  4. For grades 9-12, a Drug Testing Consent form must be signed by parent & student must be turned in to the office by the end of the first week of school. This is also done during online registration. If it is not completed at registra

  5. Sports fees, which consist of a participation fee and a transportation fee, must be paid to the office before the first contest.
